If a child with a resident registration in Minami Ward is enrolled in a private or national elementary or junior high school, it is necessary to submit a “Notice of Out-of-area Study” to the Minami Ward Office.
What you need to prepare
"Notice of out-of-area attendance"
Download the “Notice of Out-of-Area Enrollment (PDF: 50KB)” and list all required items.
(For Phone number, please fill in the number that can be contacted during the day. If you have any questions, we may call you. )
“Consent Form for Enrollment (Enrollment) (Permit)”, etc.
The original document (copy not allowed) that shows that you have been allowed to enter the school you are going to attend, such as "School (Enrollment) Consent Form (Permit)".
In addition, since the "Success Notice" is not a document that is allowed to attend school, it cannot be used in the "Notice of Out-of-area Study".
