About mail submission of out-of-area attendance notice

elementary school student students who have registered as residents in Naka Ward also receive notifications by mail when they enter private, prefectural, or national elementary and junior high schools.
(International schools and various schools are not subject to this notification.)
1.How to apply
・Download the application form (PDF: 55KB) and fill in the required items in the parent (applicant) column and thick frame.
(For Phone number, be sure to enter a number that can be reached during the day. If you have any questions, we may contact you.
Please refer to the out-of-area study registration form (Entry Example) (PDF: 65KB) for details.
・You will need an original document (not a letter of acceptance) that shows that you have been allowed to attend school, such as a letter of consent from the school you plan to attend.
Please attach it to the application form you have filled out and mail it.
