Seya Ward's Ending Note "For the Future and for the Family"

The Seya Ward Ending Note was introduced by the Volunteer Club Interact Club of Hayato Yokohama High School and the Rotary Club of Yokohama Seya.
It is open to the public at the entrance of the Seya Ward Office. Check it out!

It is a notebook that looks back on your life so far and describes your thoughts on how you want to walk in your future life.
In the Seya Ward Ending Note, you can write about yourself (life events, hobbies, special skills, favorite things, asset status, etc.), intentions in case something happens, and messages to loved ones.

• I understand the intention and hope of the person
• You can meet your wishes
• I don't have to worry about what to do

The Seya Ward Social Worker's Subcommittee (Social Worker at the Seya Ward Regional Comprehensive Support Center, Seya Ward Japan National Council of Social Welfare, Elderly and Disabled Support Division, Seya Ward Office) created an ending note for the Seya Ward based on a questionnaire survey.

This is a booklet that summarizes glossary and points for entry to make it easier to write the Seya Ward version of the ending notes.
There is also information that will help you live in the future, so please use it when you start writing the Seya Ward version of the ending note or when you are unsure about it.
