If a proxy is coming to the agency, a power of attorney is required in principle. (If the agent is the same household as the person, a power of attorney is not required.)
1 Form of Power of Attorney
The power of attorney may not be in this form. Any form is acceptable as long as required items is filled out.
2 Notes
・Please bring something that can identify the agent along with the power of attorney.
・If the contents of the power of attorney are incomplete, we may not be able to proceed. Please make sure that there are no omissions.
・This style is for National Pension Plan Section Asahi Ward Insurance and Pension Division. This is different from the power of attorney sent to the pension Organization of Japan. Please refer to the website of the Japan pension Organization for power of attorney to the Japan pension Organization.
3 When it is difficult to create a power of attorney
