• We will confirm the proposal qualifications of the applicant who intends to participate in this proposal, and mail a proposal qualification confirmation result notice (Form 2) regardless of whether or not they have qualifications. If you are able to confirm that you have a proposal qualification, we will also send you a proposal-related document submission request form (Form 3).
• Notification will be given by Wednesday, July 8, 2020.
• An applicant who has been notified that his / her eligibility has not been confirmed may request a written explanation of why his / her eligibility was not confirmed. In addition, please submit document to Yokohama-shi child blue and youth upbringing section by 5:00 pm five days after excluding city hall closing day from the next day when Motoichi sent notice.
• When an explanation is requested by the preceding paragraph, we will respond in writing to the person who requested the explanation within 5 days excluding the city hall closing date from the day after the city received the document.
• If you wish to withdraw your proposal after submitting the application form, please submit a withdrawal form (Form 5).

• Please print all on one side.
