About Isogo Ward's certification procedure for nursing care (support required)

In order to use the The Long-term Care Insurance Service, you need to be certified as requiring nursing care (support required).
The application procedure for receiving nursing care (support required) certification is as follows.
About care insurance system of Yokohama-shi, please see page "summary of care insurance system" of City of Yokohama, Health and Social Welfare Bureau.

There is a validity period for certification. You can apply for renewal 60 days before the end date of the certification period (PDF: 65KB).

※It may not be accepted between noon and 1:00 p.m.

The items required to apply for certification are as follows.
※There is application for nursing care (support required) certification in ward office, each community care plaza.
　1 The Long-term Care Insurance Insured Card
　　If you do not find it, please let us know that it was lost at the time of application.
　2 Those who know the name of your medical institution or doctor
　　Example: patient's card Shinryo-ken's appointment slip

Please refer to the usage procedure for procedures after applying for nursing care (support required) certification.

Once the certification results are obtained, we will send you a notification of the certification result and a new The Long-term Care Insurance insured card.

In principle, the notification of the results of the certification and the The Long-term Care Insurance insured card will be sent to the registered location of resident certificate by registered mail.

You may be able to change the destination.

Documents required for receipt of proxy/agent
・Power of Attorney (PDF: 104KB)
・Identity verification materials of the person (delegator) (copyable)
・proxy/agent's identity verification documents (original)
　

If you are applying for certification and have not yet obtained the certification result, you can withdraw the application.
・The physical and mental condition is not stable and visits cannot be conducted.
・I cannot go to the hospital and cannot prepare a written opinion of family doctor.
・I can't use the The Long-term Care Insurance service.
