1 Application for certification for long-term care (support required)

Please fill in required items and submit it with your The Long-term Care Insurance insured card.
Notes
Applications for renewal applications will be accepted 60 days before the expiration date of the certification validity period.
2 Home care service and care prevention care management plan creation request form

Please write required items and submit it with your The Long-term Care Insurance insured card.
The attached certificate of insured person will be returned by registered mail to the registered location of resident certificate on the next business day.
3 Information Provision Application Form

Please fill out the application form with required items and prepare the attached documents as necessary.
It will take approximately one week from application to offer. Thank you for your understanding.

B. Applicant Confirmation Documents
Identity verification documents (employee card, employee ID card, etc.) with a face photo with a description of affiliation
In the case of identification documents without the name of the office, a certificate of enrollment is required.
C. Contract Documents
If the home / care prevention service plan creation has been notified in advance, it can be omitted.
If you enter the facility, you need a copy of the contract. (Only the stamped part is acceptable.)
(a) When you apply at the counter
We will check the applicant confirmation documents twice at the time of application and at the time of delivery of a copy.
Either one must bring the confirmation documents of the applicant (applicant or representative on the front of the application form).
(b) If you wish to apply by fax
There is a temporary application form for fax only. For more information, please contact us by phone.
When you receive the materials, please bring the original information application form and the applicant confirmation documents.
(c) When applying by mail
If you wish to receive a copy by mail, we will send it by registered mail.
In addition to the above required documents, please attach a return envelope (with stamp) and mail it.

Notes
- Those who use the home service cannot provide it if there is no notification of the home care service plan creation request form. Please be sure to check before applying.
- For contractors of the preventive plan, please apply via the Regional Comprehensive Support Center in charge. (If you wish to apply directly, please contact us separately.)
- Please return the provided materials immediately after the end of the contract.
