(Renewal application)
・Even if you have already been certified as requiring long-term care, you will need to apply for renewal before the validity period expires to continue using The Long-term Care Insurance services. You can apply for renewal 60 days before the expiration of your certification validity period.

• The Long-term Care Insurance Insured Card (issued at the age of 65)
• Those who know the name of your medical institution, doctor, etc.

• Medical insurance insured person's card (* Only for those aged 40 to 64, original or copy)

　※One of "valid medical insurance insured person identification", "qualification confirmation", "notice of qualification information".

If you are eligible for the Address Special Exception System (Note 1), please check the “Insurer Number” on the The Long-term Care Insurance Card and submit it to the Elderly and Disabled Support Division of each ward.

Note 1) The special system for the Address area is a system in which an insured person in The Long-term Care Insurance enters a nursing care insurance facility or a paid nursing home in another municipality from the municipalities where they live and transfers resident certificate to the location of the facility.
