Documents to be submitted
- Vaccinations English Certificate Issuance Application Form and Reference Materials for Vaccination History (Please fill in the application form above)
- A copy of documents (in principle, the Maternal and Child Health Handbook) describing the history of routine immunization of children
- Identification documents confirming the name, date of birth, Address, etc. of the inoculated person (not required to submit a copy, please see it at the counter)

About 2 to 4 weeks (depending on the situation at the time of application)

- It is necessary to come to the agency once each at the time of application and receipt.
- Upon completion of the issuance of the certificate, the applicant will be contacted by telephone.
- When you receive the certificate, please bring your identification documents to the counter.
- Upon receipt of the above, you will receive the certification fee at the counter. Thank you for your cooperation so that no change will occur.
