What do I need to apply for The Long-term Care Insurance?
A
You can bring your The Long-term Care Insurance Card (65 years old or older), employee health insurance Card (40 to 64 years old), your medical institution name, location, and family doctor name, and patient's card Shinryo-ken and private seal (not required if you are here).
