Infant health checkups are an opportunity to confirm the growth of your child at the milestone of growth and support child care, and it is important to have them take them at a better time.

We will notify you in advance. Please come at the date and time indicated in the notice.
Please fill out the enclosed medical questionnaire and bring it with you.

（１）　If you have a cold, a fever of 37.5 degrees or higher, or an acute rash, please refrain from consulting.
（２）　Please refrain from consulting not only for children who are subject to medical examinations but also for those who have symptoms such as fever or cough in their family.
（３）　Thank you for your cooperation in taking out used diapers.
