Certificates required to change Address and permanent domicile

Certificate of change to Address

The "Notice" issued by the New Address will be a certificate for the change of Address.

Mailed to the head of household around mid-September 2017
Six cards are issued per person aged 16 or older living in the district.
If the number is insufficient or if you are under 16 years of age and need a certificate, after Monday, October 23, 2017, at the 202 window on the 2nd floor of the Izumi Ward Office, please request a "Certificate of Change of House Indication" (free).
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office work center.

Certificate of change to permanent domicile

The "permanent domicile Correction Notice" will be a certificate for permanent domicile's change procedures, such as rewriting your driver's license permanent domicile.

Mailed to the first person in family register around late October 2017 (after October 23).
Issued one copy to family register, which has a permanent domicile in the district.
If the number is insufficient, please request a "Certificate of Change of Land Name (permanent domicile Change Certificate)" (free) at the 202nd floor of Izumi Ward Office on Monday, October 23, 2017.
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office work center.
