Certificate of change to Address
The "Notice of Change of Residence Indication" representing the new Address is the necessary certificate for the Address change procedure.
・Mailed to the head of household around mid-September 2020
・Six cards are issued per person aged 16 or older living in the district.
If the number is insufficient or if you are under the age of 16 and need a certificate, please contact the Yokohama City Mail Request Office after Monday, October 19, 2020 (free of charge) Please request). Certificate of change to permanent domicile
The "permanent domicile Correction Notice" is a certificate required for procedures for changing permanent domicile, such as rewriting your driver's license permanent domicile.

About procedure of my number card (personal number card)
Those who have a My Number Card (Personal Number Card)
The Address notation will be changed due to the indication of residence in the Nishiya district of Hodogaya Ward, so it is necessary to change the Address on the card face. Please proceed with the procedure while looking at the house display bookmark (PDF: 2,975KB) (page 8).
In addition, Address on My Number Card is an old notation, but you can use it as a personal identification document by combining the notice of change of residence indication.
