Request a copy of the family register's tag at the counter
This is a procedure for obtaining a copy of the family register tag at the counter. It can be obtained at the ward office or administrative service corner. family register's tag is a certificate issued at the place of residence along with family register and contains the history of Address since family register was created.

Billing
- A copy of the family register's tag in Yokohama can be requested at any ward office Family Registry Section Administrative Service Corner of any ward in the city.
- You can also request it by mail.
- If you have a permanent domicile in the city and have a My Number Card (Personal Number Card), you can request it at a convenience store or online.

The person, etc.
Person (person) listed in the name column of family register
If you are between the people listed in the same family register name field (e.g. husband and wife), you can claim it as yourself.
Spouses, direct ancestors (parents, etc.), direct descendants (children, etc.) of the above persons
Please fill in the invoice with the person listed in the name column of family register.
Example: An example of a relationship when Taro's grandson requests a family register with Taro in the name column (the claimant is a child of "Sakura" who is a child of "Taro")

A power of attorney created by the person, etc. is required.
Third party
If the request is made by a third party other than the principal, it is necessary to clarify the name of the recipient, the indication of family register (permanent domicile, the first person) and the purpose of using a copy of the family register tag. In addition, we may ask for materials (such as a copy of a contract) that can confirm that the request is for justifiable reasons.

- A person who is necessary to exercise his or her rights or fulfill his obligations
- A person who needs to submit to a national or local government agency
- A person who has a justifiable reason to use the items described in the family register tag
- Specified clerks (lawyers, etc.) requested by persons A to C above
What I need
1. Invoices for family register Certification, etc.
- It is available at the counter.
- We can download from page (outside site) of Yokohama-shi electronic application, report service beforehand.
2. Identification documents of the person coming to the counter
- Driver's license
- Passport (passport)
- My number card (personal number card) or Basic Resident Register card with photo
- Resident Card or Special Permanent Resident Certificate
Notes
- If you do not have the above documents, you will be asked to confirm your identity at the counter by asking multiple questions such as health insurance card, pension notebook or basic number notice.

3. Power of Attorney (in the case of proxy/agent)
- Please refer to the sample by the person listed in the name column of family register (or its direct ancestry/direct descendants).

4. Documents confirming the authority to claim (other than those listed in the name column of family register to be claimed)
When making a request by a third party, you need materials that can confirm the authority of the request. For more information, please check with the ward office Family Registry Division.

Notes
Uncomputed copies and abstracts of family register cannot be handed over on the spot outside Hours of the ward office.
Fees
300 yen per copy
※You can use cash or electronic money (transportation, WAON, Rakuten Edy, nanaco) to pay the fee.
Notes
- Fees may be exempted according to laws and regulations, such as when submitting to the pension office to receive pension.

- In principle, the display of “permanent domicile Name” is omitted. If you would like to see the product, please fill out the invoice.

About remodeling
In Yokohama, on July 19, 2008, along with the remodeling of family register, the family register tag was remodeled. family register's tag (remodeled original tag) before remodeling was discarded on February 23, 2015 because the storage period has passed.
