Please fill out required items on the child birth Contact Form (postcard in the Maternal and Child Health Handbook) and mail it to Health and Welfare Center or submit it directly to the 28th floor on the second floor.
Based on the child birth contact form you submitted, we will do "Hello baby visit" and "Mother and child visit".
