Must arrive at 5:00 pm from Tuesday, August 10, Reiwa 3 to Wednesday, August 11, 2011
※Please bring it to the Tsuzuki Ward   Office Regional Promotion Division or submit it using a record-keeping method (registered registered mail, etc.). (Must arrive within the submission period)
※Tsuzuki Ward   Office Hours: From 8:45 am to 5:00 pm excluding Saturday, holidays and public holidays
[How to submit]
Arrange the application documents in order from Form 1, attach the cover, and submit 11 copies (of which one is the original).
※ For Form 11 “List of Names for Officers, etc.”, please submit one copy of the original (copy not required) and the data of the form (CD-R, etc.).
