Documents to be submitted (Form)

Please submit the following documents by the application deadline.
1. 2023 Yokohama-shi maternity helper and childcare support helper dispatch business consignment application (Excel: 14KB) (required)
2. A copy of the Yokohama City Maternity and Postpartum Helper Dispatch Business Qualified Person registration form (No. 6 style) (Excel: 13KB) and a license certifying qualification (* Only when applying for a maternity helper)
3. Yokohama City Childcare Support Helper Dispatch Qualified Person registration form (No. 8 style) (Excel: 14KB) and a copy of a license certifying qualification (* Only when applying for a childcare support helper contract)
4. Copy of designated letter of advice of visiting care establishment based on rule of Nursing Care Insurance Law ※Only applicable businesses (expiration date falls under the default period). If the renewal procedure is in progress, a copy of the application form etc. )

　　Please submit by mail or by bringing.
　　※In the case of bringing, between 9:00 am and noon on weekdays and between 1:00 pm and 5:00 pm
　　Please hand it to the staff at the Child and Youth Bureau Regional Child Care Support Section (13th floor of Yokohama City Hall).
　　In the case of mailing, please use registered mail and send it to arrive by the deadline.
