What should I do if my certificate of the physically disabled is lost or damaged?

・Please apply at the ward office Elderly and Disabled Support Division of your ward or the Child and Family Disability Support Section. You can also proceed with proxy/agent.
・We also accept mail only for applications in case of loss or damage. However, the place where the notebook is issued is the window of the ward office.

What I need

・You'll need a photo of your face.
・In the case of reissuance due to damage, please bring a damaged certificate of the physically disabled. (In the case of mailing procedure, please return at the time of ward office grant.)

[Notes on Photos]
1. A photo of the upper body taken recently (within 1 year). 4cm long x 3cm wide.
2. Wearing hats, sunglasses, masks, and Polaroid photos are not allowed.
3. Photos with no background as much as possible, such as ID photos. It doesn't show any other person.

Issuance of notebooks

If there is news of notebook issuance, please bring the news and receive notebook at ward office Elderly and Disabled Support Division or child and family disability support section.

Others

・You can apply for a reissue even if there is a problem due to the old photo in the notebook.
