Please apply at the ward office Elderly and Disabled Support Division or Children and Families Support Division in your ward. proxy/agent can do the procedure.

A personal number (My Number) is required for the certificate of the physically disabled.

[Notes on Photos]
・A photo of the upper body taken recently (within 1 year). 4cm long x 3cm wide.
