When moving, moving in, or moving out, it is necessary to carry out procedures at each local government.

・Please apply at the ward office Elderly and Disabled Support Division or Children and Families Support Division of your ward. You can also proceed with proxy/agent.
・You will need your current nursing notebook.

・Please follow the same procedures as the new application at the ward office Elderly and Disabled Support Division or Children and Families Support Division of your ward. In addition, if you can utilize the test results of your notebook before moving in, you do not need the test.
・You will need one photo of your face taken within 6 months (4 cm long, 3 cm wide, with a clear face, sunglasses and Polaroid photos are not allowed).
・Please bring your nursing notebook which you have used before.

・Please go to the ward office Elderly and Disabled Support Division or Children and Families Support Division of the ward where you moved out of the city with your nursing notebook.
・Please complete the application procedure at the transfer destination.
・Please show the nursing notebook of Yokohama City when you file at the transfer destination.
