I lost my private seal registration card.

Please report the loss to the ward office where you live. You cannot submit a report at the Administrative Service Corner.
If you have lost your private seal registration card and need Seal Registration Certificate, you will need to submit a report of the loss of your private seal registration card and apply again for the Seal Registration.

（１） When the person only reports the loss of the private seal registration certificate

• Please bring your registered private seal and identification materials.

（２） When proxy/agent only reports the loss of private seal registration certificate

• Please bring your registered private seal, power of attorney, proxy/agent's seal and proxy/agent's identification documents.
• When proxy/agent submits a notification, the document inquiry method is required.

（３） When the person submits the Seal Registration at the same time as the loss of the private seal registration certificate.

• Please bring your registered private seal and identification materials. (If you wish to change your registration seal, please bring the newly registered private seal as well.)
• Document Inquiry Method
  After the reception, an inquiry will be sent to the person by mail from the ward office. You will need to stamp your Address, name and registered private seal on the inquiry you received, and come back to the agency.
• License method
  If you present a personal identification document issued by a public office with a face photo within 10 years of issuance (driver's license, passport, etc.) within Seal Registration Prefecture on the same day.
• Guarantor method
  If the person who is in Seal Registration writes Address, name and date of birth in the warranty section of the Seal Registration application form and seals the registered private seal, Seal Registration will do so on the same day. If the guarantor is registered as a resident outside of Yokohama, you must attach a Seal Registration Certificate within three months after issuance.

（４） When proxy/agent submits the Seal Registration at the same time as the loss of the private seal registration certificate
