In Yokohama City, as a place where midwives can directly visit homes for those who have difficulty going out and learn how to raise children while receiving various advice such as nursing and childcare, Yokohama City Postpartum Maternal and Child Care Business (visit type mother and child care) is implemented. Those who have resident registration in the city are eligible.

The usage period is for mothers and baby/infant within one year of birth as of the date of use.
In addition, those who apply for use during March 2025 will be decided to use "less than 4 months after childbirth". Please note that if you wish to use it after 4 months, you will need to apply for the following change.

※For those who gave birth between April 2024 and March 2025, apply for the use of visit-type maternal and child care, and if the usage period has already been determined to be "less than 4 months after childbirth", You can apply for a change of the period to "within 1 year after childbirth".

・No application has been made before　→　Please apply for normal use (If you apply for use during March 2025, it will be decided to use "less than 4 months after childbirth")

☆★If you wish to apply for a change, please do not apply for a new use.　The URL of the application for change will be posted on this page from April 1, so please wait.★☆
・The maximum number of times of use is 3 times per delivery. Those who have already used it three times are not eligible.
　(Example: If you have been using home-based maternal and child care twice before 4 months after childbirth, you can use the remaining one within one year after childbirth after applying for a change in the period of use. )
・After application for change, we perform examination in Yokohama-shi and send notice of use change approval to home. A notification will be sent within 7 days from the date of receipt of the application.
　At the time of use, this notice is required, so please apply for extension with plenty of time.

Applicable to baby/infant and their mothers who are less than 4 months old as of the date of use who have resident registration in the city and who fall under any of the following.
（１） Those who have mental or physical disorders or childcare anxiety after childbirth
（２） Persons who cannot receive sufficient support from family members

※Please pay the usage fee directly to the midwife who visited your home.
[Notes]
・Those who need medical treatment due to illness or injury at the time of application cannot use it.
・For cancellations, please contact your midwife until 17:00 the day before use. Please note that an obstetrical teacher is expected to pay a cancellation fee (800 yen) directly when there is not any contact.

（１） Application for use
　　Before application, please confirm use rule (PDF: 123KB) of Yokohama-shi postpartum mother and child care (visit type mother and child care).
　　If you agree with the contents of the rules of use, electronic application (outside site) ※Do you do it?
　　Or send the application form (PDF: 125KB) to Child and Youth Bureau.
　　Please note that the application will be made after giving birth. We do not accept applications before giving birth. Please be careful.　
　　※For electronic application, account registration of Yokohama-shi child care support site "Pamatoko" is necessary.

（２） Examination of application
　　After receiving the application, City of Yokohama, Child and Youth Bureau will conduct a review and review.
　　A notice of use approval or disapproval will be sent to your home within 7 days from the date of application acceptance. (Ended letter)
　　※Please note that the arrival of the sent items may be delayed depending on the postal delivery situation.

（３） Acceptance of use approval or disapproval notice
　　When you receive the notice of use approval, please call the use adjustment window (Yokohama City Midwives Association Secretariat) and tell us your hope for use.
　　Adjust the date and time of the visit and the midwife to visit.

（４） Coordination with visiting midwives
　　After adjusting the use at the reception desk, the visiting midwife will contact you by telephone. Please check the items to be prepared on the day.
