At the rabies vaccination collective injection site, a triple application form for the rabies vaccination completed vote in Yokohama is required.
Please register your dog in advance before visiting.
(You may need to check microchip information, so please contact each ward office Health Sanitation Division in advance.)
※Registration procedures for dogs using the Yokohama City Electronic Application and Notification System are from application to completion of procedures.
Please note that it takes around three weeks (until a dog's license tag and Yokohama-shi rabies vaccination finished vote grant application arrives).

At the rabies vaccination meeting venue, a three-part "Yokohama City rabies vaccination completed vote grant application form" is required. The application form will be sent from mid-March to late March for those who have registered a dog in Yokohama City. On the day of using the venue, please fill out the medical questionnaire on the front of the application form (PDF: 241KB) and bring it without separating it.
（１） If you can't hold your dog or get excited, or if you can't line up in place, consider vaccinating at a veterinary clinic.
（２） Payment method is only cash. In order to shorten the time at the venue, please prepare a fee so that there is no change.
（３） There is no parking lot except for some venues. Congestion is expected even at venues with parking lots, so please refrain from coming by car.
（４） Depending on the physical condition of the dog on the day, injection may not be possible.
Especially if you have any of the following dogs, please consult with your veterinarian in advance.
Oh, I have developed symptoms of suspected allergies within 2 days after receiving the vaccination.
B. Symptoms suspected of allergies, such as a rash on the skin due to the administration of the drug.
C) Currently, they are worried about their health, such as lack of appetite or energy (especially elderly dogs).
D) Excessive excitement at the time of injection (especially elderly dogs).
E There are underlying diseases such as heart disease, kidney disease, liver disease, and blood disease. In addition, there are other diseases currently under continuous treatment.
F. Have had a convulsion seizure within a year.

The venue will be crowded for about 30 minutes from the start time. Please cooperate with the visitors avoiding crowded hours.

The group injection site may be canceled.
At 8:00 a.m. in the morning and at 11:00 a.m. in the afternoon, if weather warning (heavy rain, flood, storm, storm, heavy snow) is announced or if tsunami warning and alert has been announced, the group injection site will be canceled.
In addition, even if a warning has not been issued, the collective injection site may be canceled if there is a concern about ensuring the safety of the venue depending on local conditions.
