About sending of identification card for insured medical care for the elderly to be used from October 1, 2022

　We will send you a certificate of insured medical care for the elderly, which will be used from October 1, 2022, by September 2022.
　We will deliver it on the following schedule, so please check it.

[Delivery period]
　From Thursday, September 8, Reiwa 4 to Thursday, September 22, Reiwa 4

• During this period, we will deliver it to your home in order.
  If you do not receive it by Thursday, September 22, please contact Minami Ward Office Insurance and Pension Division (Ward Office 2F, 19th window, TEL 045-341-1128).
  
• The insurance card will be sent one by one, so even if you are a couple or a family member of the same household, the delivery time may differ.

[What should be careful about when delivering]

• The insurance card will be sent by "registered registered mail" or "no transfer required" to confirm Address.
  
• If you are transferring mail outside of your home, please go to the post office to cancel the transfer in advance.
  It will take about one week to complete the procedure at the post office.
  
• If you are unable to receive mail at home, you may be able to change the address to your family.
  Please contact Minami Ward Office Insurance and Pension Division (19th window on the 2nd floor of the ward office, TEL 045-341-1128).
  
• If the post office asks you for your insurance card and is absent, you will be mailed an "absence notification slip".
  One week after delivery, your insurance card will be kept at the post office, so please contact the South Post Office.
  If you have passed a week, your insurance card will be returned to Minami Ward Office.
