Application for medical and other qualification licenses

Health Sanitation Division is accepting new applications for licenses, registration corrections, renewal grants, and re-grant applications.

Doctors, dentists, pharmacists, public health nurses, midwives, nurses, radiologists, clinical technologists, hygienic technicians, physiotherapists, occupational therapists, orthoptist, corpse auto anatomy qualification application, registered dietitian

Associate nurse (*), prenatal control instructor, nutritionist, cook, drug handler, blowfish kitchen knife, confectionery hygienist, cleaning teacher

※Only for associate nurses, licenses issued by other prefectures are also available.

Those who live in Yokohama City
(About public health nurse, midwife, nurse registration correction, renewal grant application, one with work place in Yokohama-shi)

• In principle, the application by the person who intends to obtain (or already has) a license is required. If you have difficulty applying, please contact us in advance.
• In particular, when applying for re-grant, applications from anyone other than the person will not be accepted in order to prevent false applications by malicious third parties.
• Please bring your driver's license, health insurance card, etc. as a certificate that can identify you and come to the agency.
• The fee for the Kanagawa prefectural governor's license will be paid at the financial institution window (there is no bank in the ward office that can be paid). For this reason, please come to the agency with plenty of time so that payment can be completed within the business hours of the bank.
• Only in the case of application from the person himself, we will accept delivery (receipt) of the license by mail. (If you apply for proxy/agent, mail delivery will not be accepted.)
• If you would like to receive it by mail, please bring a certificate that can identify you, such as a driver's license or health insurance card, and come to the agency. In this case, stamps, envelopes, and cardboard (for preventing double-folding) required for mailing will be borne by the applicant, so please prepare in advance.
