In Yokohama City, there is one local activity center for the disabled in each ward. For those who have sequelae such as cerebrovascular disease (cerebral infarction, cerebral hemorrhage) living in the city, provide opportunities for interaction, light work, recreation, etc. with friends with the same disease to expand living areas and participate in society. 1 As a first step in increasing the number of out-of-home locations outside of hospital visits
Rehabilitation classroom
The rehabilitation classroom develops cerebrovascular disease and has completed rehabilitation at the hospital, and those who have just been discharged from the hospital gather to maintain and improve their physical condition while interacting with friends, and expand the scope of activities. It is held for the purpose.
(1) Target: Those who live in the city and are generally between the ages of 40 and 64 who can go to the venue by themselves
(2) Schedule Every Wednesday from 13:30 to 15:00 (about 2 to 4 times a month, about 35 times a year, in principle 1 year)
(3) Venue: Futatsu Bridge Second Community Care Plaza (469 Seyamaru, Fureaikan, Futatsubashi-cho, Seya-ku) and others
※The venue is subject to change depending on the content of the program.
(4) Costs: Free of charge
(5) Contents: Light sports experience in groups, language rehabilitation, music recreation, bus getting on and off training, etc.
(6) Flow of Application
Step 1 Consultation

Step 2 Visits
Public health nurses and nurses at the ward office visit your home and talk about your family, such as the physical and living conditions of the target person, the progress of treatment, etc. . At that time

Step 3 Determination of Participation
We will inform you about your participation in the rehabilitation class at a later date.
(7) Other supplementary items
- Consultations are also accepted from supporters (care managers, hospital counseling room staff, etc.) in addition to the target person and their families.
- Please be sure to participate after obtaining the consent of family doctor.
- If you do not have a stable medical condition, you may be refused use.
- Even if you refuse to participate in the rehabilitation classroom, please continue to consult if you have any questions about maintaining or improving physical fitness or if you have any problems in your life.
2. Activities with peers, regular outings
One Step Seya (Local Activities Center for Persons with Disabilities)
For those who have sequelae due to cerebrovascular disease, etc., have the opportunity to go out regularly while living at home, and aim to live independently in the local community through creative activities, sports, health management, community exchange, etc. It is a place to work.
(1) Those who can use it: Those who live in the city and are generally between the ages of 40 and 64 who can go to the venue by themselves

(2) Venue: One Step Seya (295-35 Futatsubashi-cho, Seya-ku, 1st floor, Sugimoto Building)
(3) Date and time: From Monday to Friday from 10:00 to 15:00 (Participation dates and frequency of participation will be determined after consultation)
(4) Costs: 1,000 yen/month
(5) Contents
- Creation of independent products such as beads, craft (paper) tapes, knitting, etc.
- Sports (Boccia, caroling, etc.)
- Exchange with friends and local communities, sale of voluntary products at local bazaars, etc.

Specialized consultation for persons with higher brain dysfunction
Diagnosis, rehabilitation, daily contact, life and work ingenuity, social participation, etc. for symptoms such as "inadvertent mistakes", "angered", "easy to forget" caused by brain damage We respond to consultation.
(1) Target: Persons with symptoms such as unmotivated, forgetable, and angery after a stroke or trauma, their families and support staff
(2) Location: One Step Seya (295-35 Futatsubashi-cho, Seya-ku, 1st floor, Sugimoto Building)
(3) Consultation date: Twice a month (reservation required) including the first Thursday afternoon (between 14:00 and 16:00).
※In addition to the regular day, a separate day consultation day is provided. Please contact us as the schedule will be adjusted according to your convenience.
