●Overview of the System
Among the so-called incurable disease where the cause is unknown and no treatment method has been established, the diseases specified by the Minister of Health, Labor and Welfare are called "designated incurable disease" and currently 333 diseases (as of July 1, 2019) are designated.
In order to reduce the burden on medical expenses on patients with the disease, we will support a portion of medical expenses for those who meet the certification criteria.
In order to receive medical expenses's payment, you must apply and be certified.
●Those who are eligible for the system
Those who fall under all of the following
1 Patients whose current address in resident certificate is in Yokohama City (if the patient is under 18 years of age, Address on resident certificate of the patient is in Yokohama City)
2 Those who have public medical insurance such as the National Health Insurance or Health Insurance Union or who are receiving social security
3 Those who are affected by the designated incurable disease and meet the certification criteria ([diagnosis criteria] and [severity] of diseases specified by the Ministry of Health, Labour and Welfare).
●Contents of medical care benefits
medical care benefits is limited to those conducted at designated medical institutions under the Act on Medical Care for Patients in incurable disease (Act No. 50 of 2014).
Medical institutions under the incurable disease Act are designated by prefectures and government-designated cities.

・Medical expenses for injuries and illnesses not caused by insurance medical treatment or the designated incurable disease
・Medical treatment at hospitals, clinics, pharmacies, and home-visit nursing stations that are not designated medical institutions

・medical expenses who went to a hospital that is not a designated medical institution (in incurable disease) (the prescription is not covered, so the cost of medicine is also excluded.)
In addition, please note that even medical institutions with designated doctors are not necessarily designated medical institutions. )
・medical expenses and drug charges incurred outside the validity period indicated on the recipient ID
・medical expenses and drug charges for treatment other than certified diseases and accompanying injuries (colds, caries, etc.)
・medical expenses, non-insurance medical treatment not covered by public medical insurance
・In-hospital meals (however, social security recipients are eligible)
・Non-insurance charges for bed charges, sheets, diapers, TVs, etc. paid during hospitalization
・Amount to be refunded from each public health insurance under the high medical costs System
・Transportation expenses, labor, commissions, etc. that are not covered by insurance paid to medical institutions at home visit fees, etc.
・Certificate fee (document fee) for clinical survey personal card etc.
・Cost of assistive devices such as glasses, corsets, wheelchairs, etc., and treatment assistive devices
・Costs for acupuncture, kyu, anma and massage
・Day care (day service (elderly day care)), home care service
●If you present a certificate of recipient of a specific medical expenses (designated incurable disease) at a designated medical institution
・medical expenses's window burden is reduced to 20% (20% and 10% remain the same).
・The monthly sum of the burdens at multiple medical institutions (hospitals, pharmacies, home-visit nursing, etc.) consulted for the treatment of designated incurable disease will be added up to the maximum monthly co-payment indicated on the recipient ID. . There is no distinction between outpatients and outpatients.
