（2） A person who has not received the nomination suspension measures based on the City of Yokohama, Medical Care Bureau Hospital Administration Headquarters Nomination Suspension Measures Guidelines on any day between the deadline for submitting the application form to the opening date of the bid.
（3） Persons who have been registered in the event "330: Waste Disposal" in the Yokohama City General Competitive Bidding Qualified Person List (related to goods, consignment, etc.) in Reiwa 5.6, and detailed "C: Industrial Waste Collection and Transportation", "D: Industrial Waste Intermediate Treatment / Final Disposal", "E: Specially Managed Industrial Waste Collection and Transportation", and "F: Specially Managed Industrial Waste Disposal" Must be registered.
（4） Being a small and medium-sized business in the city.

（1） In this case, the minimum limit price [rate] will be applied.
（2） In the tender form, enter the amount equivalent to 100/110 of the expected contract price, regardless of whether you are a tax-exempt company or a tax-exempt company pursuant to Article 9, Paragraph 1 of the Consumption Tax Law. In determining the winning bidder, the winning bid price shall be the sum of the amount stated in the bid book and the amount equivalent to 10/100 of the amount.
（3） In addition, the terms and conditions of the Yokohama City Contract shall be applied. The “mayor” shall be read as “hospital business manager” and the “Yokohama contract rules” shall be read as “Medical Care Bureau Hospital Administration Headquarters contract rules”.
