If you don't get a card with Hama, can you get it?

It is delivered at the millennium counter on the 6th floor of each ward office Elderly and Disabled Support Division and Sogo Dept. Store (Shin-Toshi Hall) Yokohama store. Please bring your official certificate (identification documents) that can confirm your age and Address. The agent can also apply if there is a public certificate of the agent and the person (identity verification document).
What you need
Please bring your official certificate to confirm your age and Address. (My number card, health insurance card, driver's license, nursing care insurance card, etc.)
