If you lose your special ride card (Keiro Pass), you can reissue it.

Bring something that can identify you (My Number Card, Health Insurance Card, License, etc.) and come to the ward office Elderly and Disabled Support Division in your ward. If proxy/agent does so, please bring your proxy/agent identification document and the identification document of the target person. However, if you apply for a reissue, you will not be able to use the lost Keiro Pass (IC card), so please look carefully before coming. ※It takes about 2 to 3 weeks to reissue the Keiro Pass (IC card).
