I lost my Keiro Pass, what should I do?
If you lose your special ride card (Keiro Pass), you can reissue it.
In case of loss
Bring something that can identify you (My Number Card, Health Insurance Card, License, etc.) and come to the ward office Elderly and Disabled Support Division in your ward. If proxy/agent does so, please bring your proxy/agent identification document and the identification document of the target person. However, if you apply for a reissue, you will not be able to use the lost Keiro Pass (IC card), so please look carefully before coming. ※It takes about 2 to 3 weeks to reissue the Keiro Pass (IC card). Thank you for your understanding.
As of October 2023, the maximum number of reissues has been eliminated.
In case of theft
Please contact your nearest "Koban" (police box) or police department and file a theft report. The police will give you the receipt number for each report, so please write down the number, bring something that can identify you (My Number Card, Health Insurance Card, License, etc.), and go to the ward office Elderly and Disabled Support Division of your ward Please come. If proxy/agent does so, please bring your proxy/agent identification document and the identification document of the target person. ※It takes about 2 to 3 weeks to reissue the Keiro Pass (IC card). In case of fire, disaster, damage or damage
