I did not fill out required items on the application form on the back of the payment form to pay the respect for the elderly pass.

It cannot be stated or modified after payment of the contribution.
If you do not apply on the back or if the contribution is free, we will send you an application form for a valid pass from October 1 in early July, so if you wish to receive it, please submit that application form Please submit.
※In addition, payment notice will be sent to those who have applied on the back after mid-August.
We will not send you the application form in early July, so please wait.
