I would like to receive a Keiro Pass, what should I do?

If you are already 70 years old or older and want to receive a new Keiro Special Ride Certificate (Keiro Pass), you need to apply. If you are 70 years old, we will send you information individually about 3 months before you become 70 years old, so please check the information received separately from the city.

（1） Application by mail
Please print out the “Grant Application Form” posted on the city's Keiro Pass website, fill in the required items, and mail it to the ward office Elderly and Disabled Support Division in your ward. At a later date, a payment slip (only for those who pay the contribution) will be mailed to your home. In addition, a set of application documents can be mailed to your home by contacting the ward office Elderly and Disabled Support Division. If you have moved in from outside Yokohama, you will need a tax certificate issued by the municipalities in the previous Address area at the time of application procedure.

（2） Application at ward window
Please bring something that can identify you (My Number Card, Health Insurance Card, License, etc.) and come to the ward office Elderly and Disabled Support Division in your ward. We have you fill in application at ward office window and hand payment notice (only one where contribution is paid). If you have moved in from outside Yokohama, you will need a tax certificate issued by the municipalities in the previous Address area at the time of application procedure.
(When proxy/agent comes to the agency)
Please bring identification documents of proxy/agent and identification documents of the subject. In addition, in the case of application from proxy/agent, payment notices cannot be issued on the spot because personal information of the target person is included. At a later date, mail will be sent to the target person.

■Those who pay the co-payment
When you receive the payment slip, please pay the contribution by the deadline at the financial institution listed on the back of the payment slip.
After payment, the Keiro Pass (IC card) will be mailed.
