About reservation of my number card
Receipt of My Number Card and renewal of digital certificates require reservations except for some ward offices. Reservations are accepted from 8:30, 30 days before.

※It is necessary to return the original My Number card. If you have lost it, please go to the ward office in your ward office.
※2 If you live in Nishi Ward and wish to receive it at the ward office, you do not need to make a reservation, so please come directly.
Reservation ID
Please confirm your reservation ID and make a reservation using the reservation method. The reservation ID is listed on the upper left of the back of the delivery notice (postcard) received from the ward office. Please note that if the characters are written by hand or red stamp at the beginning, the characters are also included in the reservation ID.
If you do not know your reservation ID, please contact Family Registry Division Registration Section, ward office in your ward office.

If you live directly in the ward of the agency system and have an expiration date of the digital certificate before April 2024, you will receive a blue envelope, and if you are after May 2024, you will receive an expiration date notice with a white envelope .

A blue envelope
There is no reservation ID in the expiration date notice. Please check the reservation method and issue a reservation ID at the time of booking.

※I tend to be crowded in the morning in Reception hours. Also, every Monday (Tuesday after holidays) tends to be crowded all day.
Reservation by fax (*People with hearing or language disabilities)
Please send the following contents to the dial FAX for exclusive use of My Number Card (045-350-8484). If you can print, please use the fax transmission form below.
- Reservation ID (described in the delivery notice)
- Date of birth (AD)
- Reservation desired location (ward office or special center)
- Reservation date and time
- FAX number
- Email address (optional)
The reservation date and time may be full, so please enter multiple dates and times if possible.
By registering your e-mail address, you can receive e-mails from the day before the reservation is completed and the day before the reservation.
Notes on Reservation
- Since there is a reservation slot for multiple people at the same time, you may have to wait at the counter on the day. Please note that we will guide you in order.
- Please come to the special center about 5 minutes before the reservation time. In addition, you will not be able to enter after the final reception time (weekdays: 7:15 pm, weekends and holidays: 4:45 pm), so be sure to come by the final reception time.
- Nishi Ward residents do not need to make a reservation if they do so in Nishi Ward Office, but once they have booked a special center, Nishi Ward Office may not have a card. If you want to change from the special center to receive Nishi Ward Office, please contact the dedicated dial.
