What kind of procedure is required if the name or location of the payer (special collection obligation person) changes?

Please submit "location, name change of salary payer (special collection duty person) registration form" to Yokohama-shi special collection center by eLTAX or mail.

For details on how to submit, please refer to "E. F: Change of Location Name" on the "Submission of Transfer / Change Request" page.
If the payer (special collection obligation person) changes due to a merger, etc., the designated number changes to the merger destination. Please also submit "Salary income transfer transfer registration form pertaining to special collection" for those who are subject to special collection before the merger.
