Please submit "salary income transfer registration form pertaining to salary payment report" to Yokohama-shi special collection center by eLTAX or mail.
Download registration form, etc.
Please fill in the “Salary Payer” column of registration form before the transfer, and fill in the “New Workplace” column at the place of work after the transfer, and submit it from the place of work after the transfer.
