About application window of need of nursing care, support required authorization

If you need nursing care and use The Long-term Care Insurance services, you need to apply for certification for nursing care and support required.
Once you have been certified as requiring nursing care or support, you will need to apply for renewal before the expiration date to continue using The Long-term Care Insurance services. You can apply for renewal 60 days before the end of the certification period.

• Those who are over 40 years old and have a specific illness
• 65 years or older

• The Long-term Care Insurance Insured Card (for those aged 40 to 64, medical insurance card)
• Those who know family doctor, such as patient's card Shinryo-ken, a medical institution
• private seal (if someone other than the person can come to apply)

• 41st window on the 4th floor of the ward office (Elderly and Disabled Support Division)

• Local Elderly Care Management Center (Community Care Plaza, etc.)

• Monday to Saturday from 9am to 9pm
• Sunday and public holidays from 9:00 am to 5:00 pm

(Closed days are year-end and New Year holidays and facility inspection days about once a month)

※In addition to the above window, you can also request a designated home care support company or a long-term care insurance facility to submit an application.
※In the case of application at Local Elderly Care Management Center or agency application, the date on which the application was submitted to the ward office is the application acceptance date.
The certification period when certified as requiring long-term care is the start date on the application acceptance date.
