Seal Registration (proxy/agent Document Reference Method)
The procedure by proxy/agent is, in principle, a document inquiry method.
Registration takes a few days (2-3 days if there is no holiday between), so please allow enough time.
If you are in proxy/agent, please bring the following four items to the counter.
- Power of Attorney created by the person (how to write power of attorney (PDF: 116KB))
- Documents confirming the name of proxy/agent (driver's license, health insurance card, etc.)
- private seal to register
- proxy/agent's approval seal
※We will send you an inquiry to your home in the evening of the day you receive it. When you receive the documents, fill out and seal required items and bring it to the ward office within 30 days. You must fill out the documents yourself, but you can also delegate the procedures at the ward office to proxy/agent again.
※If you are currently in Seal Registration, please bring your private seal registered card with your private seal card. But you don't have to see it.
