Seal Registration (Document Inquiry Method)
You will use the document inquiry method.
It will take a few days to register, so please allow enough time.
Please bring the following two points to the counter.
- Documents that can confirm your name (such as health insurance card)
- private seal to register
※We will send you an inquiry to your home in the evening of the day you receive it. When you receive the documents, please fill in required items, seal and bring it to the ward office within 30 days. You must fill out the documents yourself, but you can also delegate the second procedure to proxy/agent.
※If you are currently in Seal Registration, please bring your private seal registered card with your private seal card. But you don't have to see it.
※The documents required by proxy/agent will be different. Please start over from the first question.
