~ To patients and families who have been treated for stroke (cerebral infarction, transient cerebral ischemic attack, cerebral hemorrhage, subarachnoid hemorrhage) at our hospital ~

In our hospital, we conduct the following clinical research. In this study, we analyze and summarize the following information obtained through regular medical care. We will not conduct any new tests for this research. If you or your family are considered to be the subject of this study and you have any questions, please contact the person in charge below. Also, please do not hesitate to contact us if you are willing to use medical information for this study. After the request, the person's information will not be used for this research and will be deleted any information already collected. There is no disadvantage from the offer. However, please note that if the information to be deleted cannot be identified or the analysis has been completed, it may not be possible to delete the information from the research data.

Those who are eligible

Hospitalized after January 1, 2002 due to stroke (cerebral infarction, transient cerebral ischemic attack, cerebral hemorrhage, subarachnoid hemorrhage) within 7 days from the onset (in the case of a transient ischemic attack, from the last seizure) Person.

Purpose and Significance of Research

The purpose of this study is to collect medical chart information from many hospitals so far, to continuously grasp the actual conditions and prognosis of treatment for stroke patients, analyze treatment results, and verify treatment guidelines for stroke patients in Japan. If this study reveals the characteristics of stroke in Japan and the actual conditions of medical treatment, it is expected that it will lead to improvements in the medical treatment system of participating institutions and proposals for medical policies regarding stroke treatment in Japan.

Medical information to be used

  • Basic information (inpatient Name of the facility, date of hospitalization, diagnosis name, severity, date of birth, age, gender, height, weight, medical history, comorbidity, oral medicine, life history, family history, blood test, COVID-19 test, etc.)
  • Image examination (head CT, head and neck MRI, carotid artery echo, heart echo, catheter vascular imaging, etc.)
  • Diagnosis information (Detailed status of each diagnosis name (cerebral infarction / TIA, cerebral hemorrhage, subarachnoid hemorrhage))
  • Treatment after hospitalization
  • Progress after hospitalization
  • Status at discharge (severeness, medication content) and progress after discharge
  • If you are informed that you do not want to use medical information for this study, you will be able to record it.
