Application for food-related business license (new) (business by car)

　The Ministry of Health, Labour and Welfare has started operation of the food hygiene application system, and since June 1, 2021, procedures have become possible via the Internet. If you use the food hygiene application system, please attach the following required documents and apply.
　The application fee will be paid at the West Health and Welfare Center Health Sanitation Division window as before. After application in system such as food hygiene application, please come to window for payment. We will investigate the vehicle at the same time as paying the fee, so please come by a business car. If you are a large car, please contact us as soon as you need to check the parking lot.
　 In addition, it is possible to apply at the counter.

※　Please note that the underground parking lot in Nishi Ward Office cannot be used only for public cars.

　Please attach the following documents to the food hygiene application system.
　

• Documents to confirm the business operator (only when applying individually)
  Official documents such as driver's license, resident certificate, residence card, etc. that confirm the applicant's individual Address, name and date of birth.
  ※　When applying for a corporation, it is not necessary to attach documents in principle to check the head office location and trade name on the corporate number search site. However, if you cannot confirm the corporate number search site, please attach a certificate of entry (within 6 months from issuance).
• Documents certifying the qualification of a food hygiene manager
  If you do not have a qualified food hygiene officer, please appoint it within 3 months.

• Overview of the structure of the facility and the facilities (Excel: 631KB)
  Please write required items on the "Business by Car" sheet.
• Business plan (PDF: 217KB)
• Manufacturing method (confectionery industry only) Reference format (Excel: 103KB)
• If there is a preparation place, etc., a copy of the business license of the facility (it is not possible to prepare at a facility without a business license)
• Vehicle inspection certificate

　We will investigate the vehicle at the same time as applying, so please come by a business car when applying. If you are a large car, please contact us as soon as you need to check the parking lot.

  ・When applying individually

  Official documents confirming the individual Address, name, and date of birth (copy permitted)

  Driver's license, resident certificate, residence card, etc.

  ・When operating as a corporation

  In principle, documents are not required to confirm the location and trade name of the head office on the corporate number search site.

  However, if it cannot be confirmed on the corporate number search site, it is necessary to show a certificate of entry (within 6 months from issuance, copyable) to confirm the corporate status.

  If you do not have a qualified food hygiene officer, please appoint it within 3 months.
