　If there is a change in the following items among the business license application items, a notification is required.

• Name and address of the applicant (in the case of a corporation, its name, location and representative name)
  ※　If the applicant has a different personality, it will be a new application, not a change. (Excluding inheritance or succession of status due to split or merger of corporations)
• Name of the facility
• Foods or additives mainly handled
• Food hygiene manager or food hygiene manager, type of qualification
• Drawings showing the structure and facilities of the facility
• Classification of HACCP Initiatives
• In the case of business by car, the car registration number of the car concerned

　※　You cannot change the type of business (licensed type of business). This will be a new application.

　If there is a change in the following items among the business notification items, a notification is required.

• Name and address of the submitter (in the case of a corporation, its name, location and name of the representative)
  ※　If the submitter has a different personality, a new report is required instead of a change.
• Name of the facility
• Form of Sales
• Foods or additives mainly handled
• Food Sanitation Officer
• In the case of business by car, the car registration number of the car concerned

　If you are applying for a business license or business notification for a facility that has changed using the food hygiene application system, please use the system to complete the change procedure. Please attach the following documents to the system.
Food hygiene application system (outside site)

• In case of change of applicant information
  Official documents that can prove the details of the change (not required in the case of a change in business notification items)

If you change the applicant Address in a private business
　　Official documents confirming the individual Address, name, and date of birth.
　　Driver's license, resident certificate, residence card, etc.
When the head office location is changed due to corporate sales
　　Attachment of documents is not required in principle to confirm the location of the head office on the corporate number search site.

　　However, if the fact of the change cannot be confirmed on the corporate number search site, a certificate of entry etc. (within 6 months from issuance) must be attached.

　If there is a change in the name of the corporation, the name of the facility, the name of the food hygiene manager, the car registration number, etc., the items on the business license or the food hygiene manager's card / certificate, rewrite it and issue a new one . Grants will be made at the counter, mail, or e-mail (in the case of a change only for the food hygiene manager).

　After confirming the notification in the system for food hygiene application, etc., the person in charge will contact you. Please come to the Health Sanitation Division window in Nishi Ward Health and Welfare Center after contacting us. In that case, please bring the following documents.

• Permitted business license (when there is a change in the items described, such as a change in the name of a corporation or the name of a facility)
• The issued food hygiene manager's slip or certificate (in the case of a change in the name of the food hygiene manager or facility)

　After confirming the notification in the system for food hygiene application, etc., the person in charge will contact you. If you would like to receive the rewritten business license and food hygiene officer's slip by mail, please send the following documents to Health Sanitation Division Nishi Ward Health and Welfare Center.

• Mail Delivery Request Form (PDF: 197KB)
• Permitted business license (when there is a change in the items described, such as a change in the name of a corporation or the name of a facility)
• The issued food hygiene manager's slip or certificate (in the case of a change in the name of the food hygiene manager or facility)
• Letter Pack Plus (with the address of delivery)

　If you only change the food hygiene manager, you will receive a new food hygiene manager vote by e-mail. The person in charge will contact you to confirm the destination address.
※　Since the business license cannot be issued by e-mail, if there is any change in the information on the business license, it will be issued at the counter or by mail.

　We will conduct a survey of the facilities. After confirming the notification on the food hygiene application system, etc., the person in charge will contact you and adjust the survey date and time.
