• Fugu kitchen knife license renewal (re-grant) application form

  It's at the window of Health and Welfare Center.

• Fugu kitchen knife driver's license (in the case of change or damage to the items described)
• 2 photos (front facing, no hat, upper body image taken within 3 months before application, 4.5 cm long, 3.5 cm wide)
• Fee 2,710 yen (At the time of application, you will receive a payment slip at the counter)

  However, if rewriting and re-grant are performed at the same time, it will be 5,420 yen.

• In case of name change

  Certificate of entry in certificate of partial family register or family register (within six months after issuance)
