• Confectionery Hygienist License Application Form

• A letter of acceptance, a certificate of acceptance or a certificate of acceptance (both original)
• family register Certificate of All Matters (certified copy), family register Certificate of Personal Matters (abstract), and a copy of resident certificate (only if you are a foreign national, your nationality) and your My Number is omitted).
  ※For foreign nationals listed in each item of Article 19-3 of the Immigration Control and Refugee Recognition Act, a copy of your passport or other documents certifying your identity (please bring the original)　
• Doctor's medical certificate (issued within one month)

　　　Those that clearly indicate that they are not addicted to drugs, hens, cannabis or stimulants.

5,600 yen (At the time of application, you will receive a payment slip at the counter.)
