How do I apply for funeral expenses with National Health Insurance?

Funeral expenses are paid to those who performed funerals for those who were members of the Yokohama National Health Insurance.

Those who are eligible
・Those who are 74 years old or younger as of the day of death and who have enrolled in Yokohama City National Health Insurance
(Even if you were enrolled in Yokohama City National Health Insurance at the time of your death, you may be paid funeral expenses from the previously enrolled employee health insurance. If paid by employee health insurance, it will not be paid by National Health Insurance. )
Belongings
・Documents showing that the applicant has performed the funeral (such as a receipt or invoice of a funeral company with an address, notice of funeral thank you, etc.)
・Deposit passbook or transfer destination can be confirmed
(In the case of Japan Post Bank, the name of the store for transfer, the type of deposit, and a 7-digit account number are required.)
・Identity Confirmation Documents
※If you want to transfer money to an account other than the one who performed the funeral, you need private seal (the one who performed the funeral and uses vermilion).
