Download registration form from the Yokohama City website, attach required documents, and mail it to your ward office Insurance and Pension Division.
What you want to send
1 Copy of the date of obtaining the qualification of the insurance you enrolled in (certificate of qualification, notification of qualification information, employee health insurance qualification certificate, minor portal screen, etc.)

2 National Health Insurance Card or if you have a qualification confirmation

※Fill in the mark of Yokohama City at the bottom right with a ballpoint pen, etc.
※If lost, indicate so in registration form.
3 registration form (You can download it from the Yokohama City website)
4 Copy of identity verification documents of the submitter (My Number Card, License, Passport, etc.)
