How do I apply for high medical costs for National Health Insurance?

If you are a member of the Yokohama National Health Insurance and fall under high medical costs, you will usually receive an application form and a petition form from the ward office Insurance and Pension Division in your ward on the 20th, two months after the consultation month.
　Upon receipt of the application form and application form, please fill in required items and mail it in the enclosed reply envelope.
　After application, it will take about 1-2 months to transfer.

※ Once you apply, if you fall under high medical costs, you will be automatically transferred to the account designated as the transfer destination at the end of the month three months after the medical treatment month. Due to changes in the timing of application and household conditions, it may be necessary to submit the application again each time it falls under high medical costs.
