Cause of red skin with CPAP mask

The mask used when introducing CPAP at our hospital is a type of nose mask that covers the nose with a silicon cushion, which is the most common mask. The head gear can be worn firmly on the head, which is excellent for stability, but if the head gear is too tight to care about mask leakage (air leak), traces will remain at the root of the nose or dermatitis may occur around the nose. Even if it becomes a little red immediately after removing the mask after getting up, there is no problem if the redness subtracts in about 30 minutes. However, if the skin discoloration continues for a long time, it is necessary to adjust the head gear and change the mask.
Skin damage caused by pressure on the skin is called "puerper ulcer".
If you are bedridden and cannot change your abdication yourself, your weight will be concentrated on the bone protruding part on your back and you will have a pressure ulcer. The same caution is required for mask compression.
There are two factors that cause pressure ulcers.
Concentration of pressure
At the protruding part of the bone, pressure is easily concentrated in the area, and the risk of occurrence is high.
Continuation of physical force such as pressure
If the concentration of pressure on the local area continues for a long time, it will cause unrecoverable skin damage.
The internal pressure "32mmHg" is a barometer for pressure ulcer care.

The internal pressure of the human capillaries is usually 32mmHg, and if further pressure is applied, the capillaries will be blocked and blood will not pass through the skin tissue. In order to prevent pressure ulcers, it is important to keep them below this number. However, in fact, it is difficult to clear this 32mmHg, and it is said that 40 to 50mmHg, a numerical value from recent clinical research, is safe for pressure ulcer prevention.
Strength for fixing the mask
The strength of tightening the head gear is "the strength that does not leak air from around the mask."
The pressure often used for treatment in CPAP is 10 cmH2O (water column centimeter) ≒ 7.4 mmHg, so if the area around the mask is in contact with the skin at a pressure range of 10 to 30 mmHg, there is no danger of pressure ulcers without air leakage. It means strength.
Use the CPAP in a comfortable state that does not tighten too much and does not leak air.
