What should I do if my The Long-term Care Insurance insured card is lost, damaged or soiled?

If you have lost, damaged, or damaged your The Long-term Care Insurance card, you can receive a new card by applying for a reissue to the ward office Insurance and Pension Division.
※When applying for re-grant due to damage or soiling, please bring your damaged or soiled insured person's card.
※If you find a lost insured card after receiving the reissue, please return the lost insured card immediately.
What you need to prepare
○If the person submitting the notification is the person or a member of same household
・Identity verification documents (driver's license, driving history certificate, my number card, passport, etc.) or health insurance card issued by the government office of the visitor.
○If the person coming to the notification is other than the above
・Identification documents of the next agency
・Identity verification documents
・A document such as a power of attorney that confirms that the insured person has delegated the procedure for reissuing the insured person's card.
