What should I prepare for applying for The Long-term Care Insurance's burden limit certification?
When applying for the burden limit certification, please prepare something that shows the amount of assets such as The Long-term Care Insurance insured card, savings and deposits.
Items to be prepared at the time of application for certification
1 The Long-term Care Insurance Insured Card
2 Those that show the amount of assets such as deposits and savings of the insured (or spouse if there is a spouse)
・Deposits and savings (ordinary / regular) require the following documents for all accounts you have.
(1) The account holder and bank name can be confirmed: passbook cover, etc.
(2) The balance of the account (deposit amount) can be confirmed: The last entry page of the passbook, etc.
(In the case of a web passbook, the corresponding page is also acceptable.)
・If there are securities or investment trusts, a copy of the account balance of securities companies, trust banks, and banks, etc.
※If it is difficult to obtain a passbook, you can also submit a copy of the front of the cash card (confirmation of the account holder and bank name) and a copy of the latest usage statement (confirmation of balance).
※If you have received social security, you do not need a copy of the document that shows the amount of assets in 2.
