I moved in from outside the city. Do I need to go through The Long-term Care Insurance?

Procedures are required for citizens aged 65 and over, those aged 40 to 64 who have been certified as requiring long-term care, and those who wish to receive a The Long-term Care Insurance Insured Card. .
※If the family members who are notified of the "move-in notification" together include those who fall under the above, the same notification is required.

What you need to prepare
○If the person submitting the notification is the person or a member of same household
Identity verification documents (driver's license, driving history certificate, my number card, passport, etc.) or employee health insurance qualification confirmation issued by the government office of the visitor.
○If the person coming to the notification is other than the above
・Identification documents of the next agency
・Identification documents of the insured person
